How Do I Participate?
Participation in the HPP Registry is voluntary. If you want to take part and you meet the inclusion criteria, you should speak to the doctor who is managing your HPP. He or she can explain the program to you and will ask you to sign an Informed Consent Form (ICF) that authorizes your doctor to enter your information into the Registry database. If you are the parent or legal guardian of a minor patient, your child will need to give written informed assent, if appropriate and required, and you as the parent/legal guardian will be asked to sign an Informed Consent Form (ICF).
You can stop participating at any time. If you decide not to take part, your decision will not affect the medical care you receive. If you would like to leave the HPP Registry, you must tell your Registry Doctor.
